Enter the name(s) of the presenter(s) who delivered the course. If there are multiple presenters, list them all. For non-course CE activities, enter the name of the individual or organization associated with the CE activity, such as the supervising individual, verifying organization, or issuing agency.
Who Should You List?
- Volunteer service → Your name or the supervising dentist/organization.
- Inspection certificate → The inspecting organization.
- Teaching → Your name.
- Committee service → The sponsoring organization.
- Publication → Your name.
- Research → Your name or sponsoring organization.
